We are a non-medical residential service agency. For individuals requiring continuous skilled nursing care, we are happy to provide referrals to appropriate medical facilities.

We meet you and your loved one (virtually or in person) to connect, share our home environment, and explore mutual fit.

3️⃣ Collaborative Assessment & Funding Navigation

Together, we review care needs and explore funding options such as Maryland Medicaid Waivers, state funding, or private pay.

4️⃣ Co-Creating the Personal Care Plan

We develop an Individualized Service Plan (ISP) with you — outlining goals, routines, and partnership strategies.

5️⃣ A Welcoming & Supported Transition

We plan move-in day thoughtfully, incorporating familiar belongings, foods, and routines to create comfort.

6️⃣ Ongoing Partnership & Communication

Through regular meetings, a dedicated family liaison, and secure communication tools, we maintain an ongoing relationship focused on thriving.
